What is a Medical Home?
A Patient Centered Medical Home is not a place, it is the way we coordinate your care with all health providers. The primary care physician's office serves as the medical home—the hub—for all of your health care needs, even outside of your primary care clinic. Your health information is securely documented which is safely and easily shared with you and other providers. Our care is scientifically proven and evidence-based to help you maintain good health.
