Home Health and Hospice

Home health care provides treatment for an illness or injury and helps our patients regain their independence. Help is available whether our patient is returning home from a hospital stay, managing a serious or chronic disease, or dealing with multiple diagnoses. Our goal is to help our patients recover and regain their independence while staying safe and healthy at home—where they want to be.

Hospice provides comfort and support services for those facing the end of life. We serve southeastern Ohio and three counties in West Virginia. Patients are generally eligible for hospice care if their provider has determined that they are in their final six months of life.

Journey Hospice

Journey Hospice has a reputation for providing superior care. Because we are a small non-profit program, our care is individualized. No matter where our patient lives, our nurses can respond to their needs within an hour. Our Hospice Medical Director is fully dedicated to palliative and hospice medicine.

All of our hospice registered nurses (RNs) are certified hospice and palliative nurses. Nurses must pass a board exam to receive this certification. To maintain their certification nurses must continue to meet educational requirements and retest. Providing excellent and expert care to our end-of-life patients and their families is what we strive to do.

Marietta

We are the only Memorial Health System affiliated Home Health and Hospice agency which gives us privileges and continuity of care unlike any other agency in the area. Because of the partnership, we are able to use a multi-disciplinary approach to hospice care that includes outstanding communication and planning with our affiliated hospitals and clinics.

We also offer specialty programs that are unique to our agency. This includes our newly developed and comprehensive End-Stage Dementia program with a specialized clinician and a non-pharmaceutical approach to those suffering from the symptoms of varying dementias. We also offer the Trees in Memory program. Lastly, as a We Honor Veterans Partner we offer many specializations in end-of-life care for veterans. In addition to our distinctive programs, Marietta Hospice, An Amedisys Partner, provides inpatient services and a well-respected local team of attending physicians who oversee patient care.

Our Hospice team offers a robust bereavement program with continued care through phone calls, mailers, counseling, grief support groups, and memorial services. All of this is part of the Medicare benefit and is zero cost to family members after the passing of their loved one.
